How do I Validate my Credit Card information?

As indicated in the Credit Card Transaction Processing Agreement, before requesting a withdrawal you could be required to complete the Credit Card Verification Form placed under 'Forms & Agreements' at the bottom of the page. Please email the completed form to verification@slots.lv.

Tips for completing and sending your form:

• Print and fill out a separate form for each credit card or bank account.
• Set your photocopier to the lightest possible setting. Make sure you save and send the scanned documents in either JPG or PDF format.
• Make sure all your supporting papers are included, as outlined at the top of each verification form.
• Complete the entire form and be sure to sign and date the form where indicated.
